Before your first visit

For your convenience you may fill out the forms prior to your appointment using the links provided. If this is not possible for you, don’t worry, you can fill the forms out here, just arrive fifteen minutes prior to your appointment.


We accept insurance plans from Delta Dental, Ameritas, BlueCross/Blue Shield and Principle. We will be glad to assist you in processing your claim, but remember that insurance policies generally cover only a portion of the total treatment cost. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company. If you have dental insurance, please be sure to bring the necessary information with you on your first visit..


For your convenience, we accept payments made with your Visa, MasterCard, Discover, and American Express.

Cash discount

Holdrege Family Dental offers a 6% discount for services paid for in cash at the time of treatment or a 3% discount for services paid by card at the time of treatment.

Financing options

To help you achieve you dental goals, we are pleased to offer CareCredit as one of our financing plans. CareCredit is a leader in patient/client financing, and has helped more than one million dental patients get the treatment or procedures they needed and wanted. We offer a plan of one year no interest and a low monthly payment to fit comfortably into almost every budget.

Office Policies

Appointments: Your scheduled appointment is reserved specifically for you. Any change in this appointment affects all of our patients. If a cancellation is unavoidable, please call the office at least 48 hours in advance so that we may give that time to another patient. If two (2) broken/missed appointments occur or two (2) cancellations without 48-hour notice, our office reserves the right to NOT schedule any subsequent appointments. We understand that it’s challenging to schedule visits with your busy schedule, so we will make every attempt to accommodate your requests.

In case of a dental emergency on the weekend or after hours, an after-hours phone number will be on the office answering machine.
